Review and sign all 45-day nursing reviews in iCM and make sure staff read required nursing reviews. • Make certain that medical services for individuals who receive services are completed as required and as delegated by the nurse and physician. • Regularly check supported individuals prescribed medications, treatments and ensure they are up to date, all orders are administered as prescribed, documented and monitored as outlined in the MTTP training from the Maryland Board of Nursing and delegating nurse. • Confirms that The Arc Baltimore’s Community Living vehicles are inspected as required and vehicle logs monthly are completed submitted by established deadline. Maintain contact with the Department of Social Services (DSS) and other providers. Provide quarterly written updates as to the child’s progress.   Provide consultation and support to foster parents. Specifically, provide psycho-education to foster parents on the clinical needs of the child, and as appropriate, provide therapeutic intervention to enhance the foster child-foster parent relationship. Visit with foster parents at least twice monthly. At least one visit to be in the home. Maintain face to face contact with each child at least twice monthly, to include time alone with each child (infants do not require time alone). Ensure each child receives proper medical care. In conjunction with the DSS, provide support and consultation to the biological families of foster children to enhance the child’s relationship with family members.  Encourage regular contact and visitation between children and family members as indicated in the case plan.  Seek to involve family members in plans and decisions regarding the child. Monitor foster homes to assure their on-going compliance with state laws and regulations; assess foster parents’ performance with them as part of annual re-certification process. Together with other professional staff, be on-call to foster parents and children on a 24 hour a day, 7 day a week basis. Complete all agency required trainings for position. Facilitate and support constructive relationships between biological and foster families. Advocate for children at children’s review board meetings, IEP meetings, and court hearings. Assist in provision of New Parent training and recruitment of foster families. Assist in the Family Education Series trainings, Foster Care specific trainings, and other therapeutic group opportunities. Occasionally transport children to and from locations on an as needed basis (i.e. visits, appointments, court, etc.). Perform other duties as assigned by supervisor.

JOB SUMMARY: The Job Coach is responsible for providing job coaching and follow-up supports to every participant referred through the Developmental Disability Administration (DDA). The Job Coach provides extensive support at the job site and in
the community to include, but not limited to personal care services such as toileting and feeding, planning community vocational exploration activities, and providing all required supports to enhance participant’s achievement of their Person-Centered Plan (PCP).

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Understands and effectively implements the day-to-day operation of the volunteer program while maintaining staffing ratios as required for Community Development Services/Community Learning Services.
  • Aids program participants by monitoring the participant’s activities to include overseeing the participants during breaks and lunch period.
  • Coordinates the arrival and departure for all participants that ride Metro Access in job club and the volunteer program.
  • Transports participants in the Lifelong Learners Program (retired seniors’ group),volunteer program and job club as needed to and from job-related activities in authorized Agency vehicles.
  • Writes daily progress notes that clearly document the participants progress toward achievement of their Person-Centered Plan goals/objectives and provides these notes to the Employment Specialist who coordinates the participant’s services.
  • Documents individual’s attendance in the volunteer program and job club classes, using program attendance records.
  • Ensures all Agency, Program, and Policy on Reportable Incidents and Investigations (PORI) policies and procedures are followed.
  • Checks in with Employment Marketing Manager daily to receive direction for the next day and meets with the Employment Marketing Manager to provide and receive feedback on performance of daily program operation.

EDUCATION and EXPERIENCE:

  • High school diploma or equivalent. Verification of foreign equivalency required.
  • One-year work experience specific to disabilities, with knowledge of case management in the field of rehabilitation.
  • Must have experience with case management of consumers with Intellectual Disability and severe developmental disabilities.

  • Remain knowledgeable about relevant assistive technology and reasonable accommodations that may assist individuals with disabilities in the workplace, and discuss with individuals supported and/or employers as appropriate.
  • Observe, report, document, and provide corrective guidance and solutions to challenges and issues that arise in the workplace.
  • Facilitate positive relationships between the individual we support and their coworkers and supervisors.

  • Communicate effectively to the individual’s entire Person-Centered Planning (PCP) team regarding the needs of the employer and the individual being supported.

  • Complete all monthly support visits for individuals on caseload to meet the requirements outlined in each individual’s PCP.
  • Enter service logs into OBI’s client management software system (Twinsis) on a daily basis.
  • Maintain accurate records of individuals’ employment details such as pay rate, benefits offered, supervisory information, etc.
  • Maintain compliance with all OBI, DDA, OHCQ, and DORS policies and regulations at all times.
  • Actively participate in regular meetings with the Workforce Development team.
  • Driving your own personal vehicle and/or an agency vehicle to provide community and/or employment transportation to the participants.   

Qualifications

  • Minimum of 1 year of experience working with individuals with developmental disabilities; strong preference to candidates having specific experience in vocational rehabilitation, employment services, sales or human resources.
  • High school diploma or equivalent required; Bachelor’s degree in a related field preferred.
  • Valid Driver’s license, clean driving record and 3+ years driving experience.

The Rehabilitation Specialist acts in a supervisory capacity overseeing Psychiatric Rehabilitation Program (PRP)services provided at multiple Goodwill Centers located  within Goodwill’s territory.  The Rehabilitation Specialist coordinates all program functions, including staff management, client/case load assignments, conducting chart and documentation reviews, and provides training for rehabilitation counselors.   

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Conducts weekly clinical supervision to all PRP staff members as well as a minimum of monthly group supervision to Rehabilitation Counselors. 
  • Provides constructive feedback on treatment, services, and rehabilitation plans. 
  • Documents all supervision and program oversight in detail, in compliance with CARF and COMAR regulations
  • Works with staff to discuss and establish goals, ensuring appropriate rehabilitation plans are developed to meet participants’ aptitudes, needs and career goals
  • Monitors staff documentation within GIC’s data management system, ensuring timeliness, quality, and accuracy of staff record keeping
  • Trains and coaches staff in best practices within the rehabilitation field such as person-centered care planning, suicide prevention, client/counselor boundaries, smoking cessation and de-escalation
  • Reviews participant files including individual rehabilitation plans, monthly progress notes, and contact notes; Follows-up on missing paperwork and establishes plans to ensure record-keeping compliance
  • Ensures program and services are in compliance with Maryland state and CARF regulations to be a mental health service provider
  • Follows and stays up to date with federal and state regulations to continue providing PRP services including coordination with Medicaid ASO (OPTUM), Behavioral Health Administration, and local health department agencies. 
  • Reviews current processes, systems and standards making recommendations on modifications to proactively avoid compliance deficiencies 
  • Assists with establishing and implementing department policies, goals, objectives and procedures
  • Work in tandem with STEP Program Manager to promote psychiatric rehabilitation services in location areas and build primary referral sources with clinical programs and businesses
  • Attends community/provider meetings to promote GIC services to community members and potential referral providers
  • Maintains a partial caseload of participants when staff caseloads are full
  • Assists and coaches staff to conduct group services and responsible for ensure that proper billing policies and procedures are being considered.
  • Attends GIC meetings and trainings
  • Complete monthly report of staff efforts and programmatic outcomes 

EDUCATION AND/OR EXPERIENCE:

  • Master’s degree in social work, psychology, human services or directly related discipline preferred; Bachelor’s degree required

  • One year experience serving in a supervisory role required
  • Prior experience working with a population with various barriers to employment such as mental illness, substance abuse, limited work history, criminal backgrounds, etc. preferred
