Position Summary: The Community Health Educator plays a vital role in supporting individuals with disabilities by facilitating access to home modifications and health-related resources. This position focuses on empowering clients to maintain independence, improve quality of life, and navigate available services that enhance accessibility and safety in their homes.

Key Responsibilities:

  • Provide outreach, education, and direct assistance to individuals with disabilities regarding home modifications and related health services
  • Collaborate with occupational therapists, contractors, and funding agencies to coordinate home modification projects
  • Support clients in navigating available grant programs, financial assistance, and community resources for home modifications
  • Attend community events and workshops to promote awareness and education on independent living and accessibility solutions
  • Maintain accurate records of client interactions, assessments, and service referrals

Qualifications:

  • Experience in community health work, social services, or disability advocacy
  • Knowledge of home modifications and accessibility solutions for individuals with disabilities
  • Strong interpersonal and communication skills with the ability to build trust with diverse populations
  • Proficiency in record-keeping and client documentation
  • Ability to travel locally for home visits and community events

Preferred Qualifications:

  • Certification as a Community Health Worker or related field
  • Experience working with home modification projects and accessibility adaptations
  • Familiarity with ADA regulations and accessible housing guidelines

Wage/Salary: $25.00/hour

Shift or Work Schedule:

8:00 am – 5:00pm M-F, some evenings and weekends, as needed
